Sharing information before the doctor's visit
We organize changes in health, daily life, family concerns and care details, and share them with the doctor beforehand so the visit goes smoothly.

Home-Visit Nursing
Your doctor may visit only once or twice a month, while home-visit nurses often come to your home several times a week.
Small changes such as eating less, sleeping poorly, or a family member growing tired from caregiving are often noticed first by the visiting nurse.
At HOPE, we make sure these observations reach your doctor and care manager.

We organize changes in health, daily life, family concerns and care details, and share them with the doctor beforehand so the visit goes smoothly.
The nurse can pass on things that are hard for you or your family to explain or bring up with the doctor.
Knowing your usual condition and progress helps the doctor look ahead. We believe this brings peace of mind to you and your family.
Not only for public health insurance users: for Long-Term Care Insurance (LTCI) users too, we join doctor's visits and care planning meetings when needed.
Home-visit nursing is more than procedures and observation. We connect you, your family, your doctor and your care manager to support a safe life at home.
One person first asked us for rehabilitation. On the first visit, the nurse checked their whole body and found pressure ulcers (bedsores) on the back and buttocks, including a deep one over the lower back.
The nurse reported to the doctor that same day and started care as instructed. After that, the team worked together:
Little by little the condition improved, and the person was able to sit longer and move around more. Home-visit nursing protects peace of mind by noticing changes in daily life and responding early.
Home-visit nursing can start on the very day you are discharged. We prepare through the pre-discharge conference, and on the day you return home, a nurse visits to check your health, your medication and the safety of your home.
We aim to give you the same sense of security at home that a nurse call button gives you in hospital.

Home-visit nursing is more than medical procedures.
"I want to stay at home." "I don't want to be a burden on my family." We support time at home lived with peace of mind, close to these feelings.
