Home-Visit Rehabilitation

Not "finished once treated", but until you can move again

Features

Helping you keep using the strength you have

Medical care does not end when treatment is done. We believe it is important to recover and to be able to move with your own strength as much as possible.

At home, things you could always do may suddenly become difficult, such as standing up or going to the toilet, which can lead to trouble eating or sleeping. Unlike a hospital, you are often alone at home, and a small problem can become serious.

That is why HOPE keeps thinking about how each person can continue to use the strength they have, so they can keep living in the home they know.

Approach

We look beyond "being able to do it"

Progress in rehabilitation shows as "being able to do more on your own." But each movement is made up of many elements:

When one part weakens, another part works too hard to make up for it. It may look fine for now, but over time pain or problems can appear somewhere else.

We look at the strain your body has carried and what may happen next, and help you find ways of moving that suit your life ahead. Not just exercise, not just massage, but regaining the movements you need to live your own way in the community.

Respiratory Care

Three certified respiratory therapists on our team

  • Three of our rehabilitation staff hold the 3-Society Joint Certification in Respiratory Therapy, a qualification for medical professionals who have studied respiratory care and passed an exam.
  • For: people using home oxygen; people with COPD or other conditions who worry about shortness of breath; people whose breathing has weakened due to intractable neurological diseases; people just out of hospital who are anxious about breathing or stamina.
  • What we work on together: checking your breathing; positions and ways of breathing that make it easier; ways to clear phlegm; moving and resting with less breathlessness; practicing dressing and bathing while using oxygen; explaining assistance methods to your family.
  • We proceed under your doctor's instructions, in consultation with our nurses, according to your condition.
Team

The strength of nursing and rehab in one team

Many physical therapists (PT) and occupational therapists (OT) work at HOPE as full-time staff.

"I feel unwell today, so I want the nurse to check me." "I fell yesterday, so I want to focus on rehab so I don't lose strength."

We can flexibly decide whether to focus more on nursing or rehabilitation, depending on your condition that day and your stage of recovery.
